Admissions All of the documents under Forms to be Filled Out must be printed out and returned to Mary Lesowske in the Office at Sisters Middle School. Please bring a copy of the student's immunization record at time of enrollment. For out of state students, please bring a copy of your birth certificate and a copy of the student's immunization record.
